Hi everyone,
Please find attached a player’s questionnaire for you and your player to fill out. The information on the form will help us create your player’s page in our year end album. (gift for each player)
You can save the document , edit it and send it to Logan (Thank you Logan) as she will be taking the lead for this project. Please see the document for more info.
It is important to send pictures and form before Feb 29th, 2020.

P.S. I will also have a few forms printed out if you prefer.
