Affidavits will be available June 1st on Data Center. Managers can get access to data center from your own league. After completing paperwork online print out copies for your records.
When teams submit their paperwork in June this is what you will need for each child and team:
New Player:Original Birth Certificate (Can keep copy in your folder after original is checkded, return original to parent) Medical Release Form Player Verification Form 3 Proofs of Residency (dated betweeen February 1 , 2018 to February 1, 2019 (one from each group ) or School Enrollemnt form (Must indicate enrollment prior to October 1, 2018). Each Team: These must be done each year Completed Affidavit - (In Alphabetic Order) Signed by President and Player Agent Map - Signed & Dated by President Team Verification Checklist listed alphabetically same order as Affidavit If you have your paperwork from last year along with all the forms all you will need is: New Medical Release Form for each player (If any documents are missing from last year the child must be treated as a new player).
