Physical Forms and Code of Conduct cards need to be completed and turned in by August 6th.
The MHSAA physical card was mailed to you...ALL INFORMATION ON THIS FORM MUST BE FILLED OUT COMPLETELY, INCLUDING PARENT / STUDENT SIGNATURES, AND INSURANCE INFORMATION, BEFORE TURNING IT INTO COACH GIARMO.
Also...Be sure to turn in your Code of Conduct card completed (also mailed to you) when you turn in your Physical form.
