Residency Verification this Saturday December 14th, 2019!

Please stop buy our Fields (1225 N. Creekside Drive) to complete your registration tomorrow between 8:30AM-10:00AM to have your residency/school (if using a school waiver) verified.
All players must be verified to ensure they are within ELL boundary.

We will need ONE document from each of the following three categories along with the player’s original birth certificate:

Category ONE
1. Driver’s License 2. School records 3. Vehicle records (i.e.,registration, lease, etc.) 4. Employment records 5. Insurance documents
Category TWO
1. Welfare/child care records 2. Federal records (i.e. Federal Tax, Social Security, etc.) 3. State records 4. Local (municipal) records 5. Support payment records 6. Homeowner or tenant records 7. Military records
Category THREE
1. Voter’s Registration 2. Utility bills (i.e., gas, electric, water/ sewer, phone, mobile phone, heating, waste disposal) 3. Financial records (i.e. loan, credit, investments, etc.) 4. Medical records 5. Internet, cable, or satellite record

NOTE: Example – Three documents from the same Category (utility bill, cable bill, and bank statement) constitute only ONE document.

Lastly, please print and complete the Medical Release Form and bring it with you to Residency verification. Link is below:
