NAVIGATING THE WEBSITE

Just wanted to give some quick information on some required forms and how to access them.

On the menu tab under  "Policies and info"  there are several forms for you to use.  These forms are required for certain things and must be downloaded, printed, filled out and then returned.

Summer Hockey Waiver.......this form must be signed before your child participates in any off season hockey activity.

BLC Check Request.......this form is used if you have spent your own personal money and are requesting reimbursement from the Blue Line Club. Example: team party

BLC Expense Reimbursement.....this form would be used if you have paid own personal money for a repair or upgrade or product purchase on behalf of the Blue Line Club. Example: shelving for concession stand

BLC Cash Receipt........this form would be used if you have collected money that needs to be deposited in the Blue Line Club account.  Example: t-shirt sales

 

To access these forms, click "policies and info".......find which is the correct form you need and click on it.  This will bring you to the handout, and you will need to click on it again, or the icon next to it.  A download window will then come up, click on "download now".  Form will open, you will then need to print form, fill it out and return it.
