***ANOTHER * REGISTRATION * DATE * FOR * JUNE***
FOOTBALL AND CHEER
LATE REGISTRATION
WEDNESDAY JUNE 19th, 6-7pm
At WHY USA Properties, 4869 S Bradley Rd #104 Santa Maria, CA 93455
Note: Late registration includes a $20 late sign-up fee
Please come prepared with the following at Registration
- $220 registration fee
- (2) copies of the child's birth certificate
- (2) copies of their report card
- (2) 4x6 photos of your football player (not needed for cheer)
- Proof of residence
- AND the football player(s) so we can weigh them.
IMPORTANT: Football players that do not sign up by this date (5/31/2013) will not be able to have a name plate on their jerseys.
NOTE: CHEERLEADERS need to provide the same documentation but do not need to be present to begin their registration process.
ATTENTION returning players/families: Unresolved financial obligations (including unreturned equipment) may impact your registration fees. Those that think they may have these issues should contact the OYFL President or Vice President to determine resolution actions. Thanks!

MANDATORY UNIFORM FITTING COMING IN JUNE!!!! Date and time TBA as we are finalizing style and accessories.

Sign-ups in June will incur a $20 late fee.

PHYSICALS
Every cheerleader must have a physical.
Completed, signed and approved PHYSICAL FORMS must be turned in BEFORE your daughter will be able to go to Cheer Camp and start Cheer conditioning. We cannot make any exceptions to this rule because of liability. Therefore, please schedule your physicals with your doctor's office soon as summer months tend to be busy.
Cheer camp is on August 3rd and Conditioning begins August 5th. Get your physicals turned in BEFORE that date!
NOTE: OYFL IS HAVING A PHYSICAL CLINIC AT SIMS PHYS THERAPY ON 7/27/2013.
Also, for your convenience we have the CCYFL Physical Form located on our website underForms/Handouts. Have your doctor complete while your daughter is at her physical.
