What to bring to tryouts?

1. Extreme Registration Form (print off – Media tab – documents). Will have extra copies at tryouts.

2. Go onto SRVA.org and either click on the link renew or new member. This will take you to Webpoint . On Webpoint – register or renew and pay the $5.00 tryout fee. (instructions under the media tab – documents).

3. Print off USAV membership card and bring with you.

4. Print concussion form (media tab – documents) and bring with you. Will have extra copies at tryouts.

5. Fill out medical release form (available on srva.org website or on media tab – documents). Will have extra copies at tryouts.
